Promni Health offers First Contact Practitioner (FCP) Podiatry services within primary care. Delivering front door specialist assessment, reducing complications and supporting long-term patient outcomes.

  • Assessment of undifferentiated foot and ankle presentations in primary care, which include: Neurovascular, Musculoskeletal, Dermatological, Endocrinological and Nail pathology
  • Early access, initial diagnosis, and risk stratification of complex foot conditions
  • Necessary management of high-risk patients
  • Direct onward referral pathways to specialists including Vascular, MSK, Orthopaedics and Dermatology
  • Integrated working within primary care MDTs facilitating clinical decision-making and reducing GP workload

  • Clinical governance, supervision, and competency frameworks for safe FCP practice
  • Service redesign and capacity support to improve access and reduce waiting times

QOF and Long-Term Condition Management

We facilitate primary care teams to:

  • Identify and code high-risk patients earlier, including those with CVD and diabetes
  • Support preventative and proactive management of high-risk patients
  • Reduce variation in care for high-need populations
  • Prevent avoidable complications, including ulceration and amputation

We support systems to:

  • Reduce avoidable complications and hospital admissions
  • Improve preventative care pathways
  • Strengthen community-based podiatry provision

How it works

Mobilisation

We work with your PCN or system to identify high-risk foot health cohorts, including patients with diabetes and long-term conditions, and design a podiatry model aligned to QOF delivery and preventative care priorities.

Deployment Roadmap

Podiatrists are embedded within primary care and community settings to deliver routine and specialist care, including diabetic foot assessments, risk stratification, and early intervention – supporting patients at the front door and through ongoing management.

Ongoing Management

We provide continuous clinical governance, supervision, and outcomes monitoring – reducing avoidable complications, improving long-term condition management, and preventing escalation to secondary care.
