Supporting QOF and Long-Term Condition Management

Our podiatry service plays a key role in supporting delivery of Quality and Outcomes Framework (QOF) indicators and improving management of long-term conditions.
We support primary care teams to:
- Improve delivery of annual diabetic foot checks
- Identify high-risk patients earlier, including those with peripheral arterial disease
- Support proactive management of patients with diabetes and cardiovascular conditions

Mobilisation
We work with your PCN or system to identify high-risk foot health cohorts, including patients with diabetes and long-term conditions, and design a podiatry model aligned to QOF delivery and preventative care priorities.

Podiatrists are embedded within primary care and community settings to deliver routine and specialist care, including diabetic foot assessments, risk stratification, and early intervention – supporting patients at the front door and through ongoing management.
