QOF and Long-Term Condition Management

We facilitate primary care teams to:
- Identify and code high-risk patients earlier, including those with CVD and diabetes
- Support preventative and proactive management of high-risk patients
- Reduce variation in care for high-need populations
- Prevent avoidable complications, including ulceration and amputation
We support systems to:
- Reduce avoidable complications and hospital admissions
- Improve preventative care pathways
- Strengthen community-based podiatry provision
How it works
1
Mobilisation
We work with your PCN or system to identify high-risk foot health cohorts, including patients with diabetes and long-term conditions, and design a podiatry model aligned to QOF delivery and preventative care priorities.
2
Deployment Roadmap
Podiatrists are embedded within primary care and community settings to deliver routine and specialist care, including diabetic foot assessments, risk stratification, and early intervention – supporting patients at the front door and through ongoing management.
3
Ongoing Management
We provide continuous clinical governance, supervision, and outcomes monitoring – reducing avoidable complications, improving long-term condition management, and preventing escalation to secondary care.
