Beth, RMHN – Health Dearne and Valley PCN
Improving access, continuity, and patient outcomes through specialist mental health input
Before the introduction of a dedicated RMHN role, patients experiencing mental health difficulties often faced long waits and limited appointment availability – particularly those with complex or overlapping needs.
By embedding an RMHN within primary care, the service has significantly improved timely access to support, while delivering more holistic, patient-centred care that considers both mental and physical health.
The Approach
Beth, our RMHN, has focused on:
- Increasing appointment availability for patients in need of mental health support
- Delivering holistic assessments that consider both psychological and physical health
- Identifying patients earlier who require escalation to specialist services
- Building continuity of care and stronger therapeutic relationships
The Impact
- Improved access to timely mental health support
- Earlier identification and referral to secondary care services
- Enhanced continuity of care for patients with complex co-morbidities
- More holistic, joined-up care across physical and mental health needs
Real Patient Impact
One patient presented with severe anxiety and depression following complications from cancer surgery in her 20s. The procedure had left her with ongoing physical discomfort and body dysmorphia.
Through a holistic and supportive approach, Beth worked with the patient to successfully apply for an Individual Funding Request (IFR) for an apronectomy.
This intervention addressed both the physical and psychological impact of her condition – demonstrating the importance of treating the whole person, not just symptoms.
What Patients Are Saying
“I finally felt listened to and understood.”
“I didn’t feel alone with it anymore.”
Looking Ahead
While formal data is still being collected, early indicators show increased appointment availability and improved identification of patients requiring specialist input.
This model demonstrates the value of integrating mental health expertise into primary care – delivering accessible, compassionate, and truly holistic care for patients.
