Supporting Families to Navigate Frailty & Future Care

Reducing GP workload and improving access through First Contact Podiatry

The Challenge

Karen was identified as frail following a stroke, with limited progression through rehabilitation services. Her family, particularly her husband, had not fully recognised the extent of her decline, making future planning and decision-making challenging.

Our Approach

Working jointly across services, we supported proactive care planning and facilitated a ReSPECT conversation with her GP. We brought together primary and secondary care teams to ensure aligned decision-making and provided space for open, honest discussions with Karen and her family.

We supported the family in understanding their role as carers and explored ways to maintain quality of life, including adapting access to activities she enjoyed such as gardening.

The Impact

The family developed a clearer understanding of Karen’s condition and future needs. Her husband was better able to process her frailty and shifted focus towards spending meaningful time together.

Care planning is now aligned with Karen’s wishes, with improved emotional support for the family as they navigate this stage.
