Between February and May 2026, Promni Health delivered a multidisciplinary work and wellbeing pathway across Bordesley East Primary Care Network, supporting patients with work-related health needs through coordinated assessment, fit note support, onward referral and early intervention.
Delivered across 3 clinicians embedded within the PCN, the service was designed to:
- Reduce GP administrative burden
- Improve access to work-focused support
- Support earlier intervention and facilitate more integrated patient management across primary care and wellbeing services.
Interim Outcomes (February–May 2026)
- 219 patient contacts managed
- 158 patients supported within the pathway
- 87 onward referrals into WorkWell and associated support services
- 3 clinicians embedded across the PCN
- Multidisciplinary coordination across physiotherapy, MSK, wellbeing and work-focused pathways
Estimated GP Capacity Released
Promni Health managed 158 attended patient contacts within the pathway.
Using a conservative estimate of 15 minutes per GP appointment, this equates to approximately:
39.5 hours of GP clinical capacity
This enabled GP time to be redirected toward more clinically complex patients, while ensuring individuals requiring work and wellbeing support received earlier intervention through a dedicated multidisciplinary service.
Physiotherapy-Led Work & Wellbeing Support
The pathway was delivered through a physiotherapy-led and multidisciplinary model focused on:
- Functional recovery,
- Return-to-work support
- Lifestyle medicine principles
- Early MSK intervention
- Workplace adjustment advice and coordinated rehabilitation pathways.
Rather than operating as a standalone fit note service, the pathway focused on helping patients:
- Remain in work where appropriate
- Access earlier support,
- Improve functional capacity and receive coordinated onward care tailored to work-related health needs
Supporting Return-to-Work and Early Intervention
Patients accessing the pathway were supported through:
- Work-focused clinical assessment
- Rehabilitation and self-management support
- Onward referral into WorkWell pathways
- Physiotherapy and MSK services
- Wellbeing and lifestyle interventions and coordinated multidisciplinary management
The service supported a more proactive approach to work and health management, aligned to emerging NHS priorities around prevention, integrated neighbourhood working and reducing avoidable pressure on general practice.

As NHS England pilots new fit note approaches nationally, there is increasing focus on:
- Multidisciplinary work and health pathways
- Reducing GP administrative workload
- Earlier intervention and prevention
- Supporting patients to remain in or return to work and improving integration between primary care, wellbeing and vocational services
The Bordesley East PCN pathway demonstrates how physiotherapy-led and multidisciplinary models can support these priorities while delivering practical operational benefits across primary care.
