Role Profile

As an Enhanced Practice Nurse, you will be embedded within a Primary Care Network (PCN) multidisciplinary team, supporting the delivery of high-quality clinical care to residents in care homes.

You will undertake clinical assessments and contribute to patient management within agreed PCN care pathways, working under the clinical supervision, governance, and escalation frameworks of the host PCN.

The role focuses on delivering structured, proactive care alongside responsive clinical support, helping to improve continuity of care, reduce avoidable hospital admissions, and enhance quality of life for residents.

This role involves regular visits to care homes within the PCN footprint and requires travel between sites.

Outcomes Expected

You will support the PCN with overriding objectives to:

  • Reduce avoidable hospital admissions
  • Improve continuity and coordination of care for care home residents
  • Support proactive management of complex, long-term conditions
  • Enhance quality of life through person-centred care planning
  • Provide clinical support to care home staff and reduce system pressure

Key Responsibilities

You will contribute to a wide range of clinical activities within agreed PCN pathways, including primary care, including:

  • Undertaking clinical assessments of care home residents
  • Supporting new resident assessments and post-hospital discharge reviews
  • Contributing to personalised care and support planning in collaboration with the MDT
  • Participating in regular care home ward rounds and MDT meetings
  • Supporting advance care planning discussions and associated documentation
  • Monitoring and supporting management of long-term conditions and clinical deterioration
  • Providing clinical guidance and support to care home staff
  • Supporting delivery of vaccination programmes in line with local protocols
  • Identifying and escalating safeguarding concerns appropriately

You will work collaboratively with GPs, pharmacists, district nurses, and the wider MDT to deliver coordinated, person-centred care aligned with patient needs and preferences.

Person Specification

Essential

  • Registered Nurse (NMC)
  • Relevant postgraduate qualification / experience at Enhanced Practice level
  • Experience working with older adults and/or within care home settings
  • Strong clinical assessment and decision-making skills
  • Ability to work effectively within a multidisciplinary team
  • Excellent communication and interpersonal skills
  • Ability to work within a community setting with a flexible approach
  • Ability to travel between care home sites

    Full clean driving licence and access to own vehicle

Desirable

  • Independent Prescribing qualification (V300)
  • Experience working in primary care or community services
  • Experience supporting care home residents or frailty pathways
  • Knowledge of NHS primary care systems and MDT working
